For the Executive Summary assignment my idea for the quality improvement initiative is to do more actual physical Hourly Rounding. Often times, we document hourly rounding completed, but we hardly have the time to do it on an hourly basis sometimes, it is impossible because of the workload we have to handle as nurses. Hourly rounding is very important and plays a big role in improving the patient experience. I want to implement a system where hourly rounding becomes stricter where the patient care technicians (PCT) have to participate and make it part of their responsibilities. Hourly rounding is very vital in patient safety because we get the opportunity to prevent falls and decubitus ulcers. During that time, patients can be repositioned and we get to change wet care pads and the patients can be bathe. At the hospital where I work, we are required to address the 5 p’s (pain, potty, position, possession, and privacy). The combination of hourly rounding and the 5 p’s is designed to meet the needs of patients and their caregivers, by improving access to timely, quality and safe healthcare. In order to have successful hourly rounding, management has to higher more patient care technicians to help the nurses, it will be cost effective in the long run. Nurses alone with a heavy load of patients cannot address the 5 p’s. There should be about four patient care technicians per unit in the hospital. Furthermore, this approach improves patient satisfaction, and reduces the rate of falls, pressure ulcers, medication errors and call light usage, and decrease patient anxiety. It also improve the HCAHPS scores.

  • Research how much it cost a hospital to treat hospital acquired wounds.
  • Research how much money they would save from hiring more patient care technicians compare to much money they waist on paying for pressure ulcers.
  • Research how Medicare/Medicaid do not pay hospital acquired wounds.
