Project Reports

Reminder:  Every student must do six hours of service each semester.  This service may not be performed within your immediate family and you may notreceive any money for the service.  After the project(s) is (are) completed, you are required to write a report about the service.  The format for the report will be specified below.  The supervisor or person for whom the service was done is asked to sign for verification of service.  If this person is unavailable to sign the paper, a parent or guardian must sign and explain why the person is unable to sign. The service project is graded as a major project.
Requirements:

1. A total of 6 hours of service (either one project totaling six hours or several projects totaling six hours)
2. Cover sheet with the appropriate signatures (note: grade will be lowered if this sheet is not turned in)
3. A written report that is 2 pages in length.
4. The following questions are to be answered in the report.
   1. What was the actual service project you performed?  Be specific and detailed.
   2. Where did you perform this service?
   3. Why did you choose this service?
   4. Who were the people who benefited from your service?
   5. What need did this service fill in the community?
   6. Was there a sense of community while performing your service?  Explain.  If not, how could community have been incorporated?
   7. What was the overall reaction of the service you provided by the community?
   8. When performing your service, which of the five core values do you think were demonstrated most (community, respect, responsibility, honesty, compassion)? In a sentence of two, explain how.
   9. How could you exhibit compassion more through your service?
   10. What insights did you gain from the experience?  Describe your feelings, thoughts, observation, and realizations.
   11. How would you relate your service experience to your personal faith and to the teachings of Jesus and the Church?  How did you encounter Jesus in the experience?  How did you live out the teachings of Jesus in your service?  How did the service grow in your faith as a young Christian?
   12. Do you see any application to this service project and to Jesus’ words: “I have come to serve, not to be served?”
   13. Is there anything you would do differently if you could do this project again?
   14. What, if anything, did you learn about yourself? Is there anything that you would change about your service to better show compassion and understanding for those you helped?
   15. Do you feel your hard work paid off in the end?  How did you remember and focus on the fact that you were offering service rather than just filling a requirement?
